It begins with a unique Consultation:

During your consultation appointment, Dr. Ishoo goes well beyond what most centers provide. His goal is to educate and inform clients with details about their options and our procedures and discuss concerns and goals to identify how to achieve optimal results. Once you have identified a concern about your skin, face, or body you can expect the following consultation process:

  • Review your medical history and any current medications.
  • Identify and discuss your goals, desires, and concerns.
  • Based on your objectives, Dr. Ishoo will determine what options you are a candidate for.
  • All relevant procedures will be explained in depth and offered.
  • Setting options for procedures will be offered (local, sedation, or general anesthetic).
  • Realistic expectations and results will be determined.
  • All of your questions will be answered.
  • If you decide that the procedure is right for you, you will be given the necessary information to form a treatment plan.

Keys to a great result are:

  • An informed and educated patient – regarding their options, procedures and the process.
  • Realistic expectations about outcomes.
  • Full disclosure of anything which could impact your treatment.
  • Patient safety and privacy.
  • Attaining a natural look.
  • Dr. Ishoo’s good sense of the appropriate aesthetics for your unique face and body.
  • Impeccable technical skills.
  • Compliance with all instructions and careful adherence to post-procedure care.

Choosing a Doctor

Choosing to have cosmetic procedures is an important decision. The so called “Medical Spas” owned and operated by a non-physician are in fact illegal especially if not registered as a medical clinic by the Department of Public Health. This registration is required even if it is partially owned by a non-physician. Physicians renting out their medical license to these businesses especially as the unqualified staff makes diagnosis and treats medical conditions with medical grade prescriptive treatments such as IPL, LASERs, injectables etc. may constitute violation of the Corporate Practice of Medicine law enforced in all 50 states and in fact is a serious conflict of interest when a physician’s employers are practicing medicine under the physician’s license motivated only by profit and the physician can never make a decision contrary to the owner’s best financial motive since they are not actually on the premises. All diagnosis and treatment planning MUST be done by a physician, not by an esthetician or a nurse. There is no such certification as a “Medical aesthetician”, “LASER technician” or “Acne specialist” recognized by any professional licensing agency in Massachusetts. In fact, any practice of medicine by an aesthetician is strictly forbidden by the MA Board of Cosmetology. By definition, if the procedure causes burning, discoloration, deep tissue collage stimulation, bleeding or even extraction of a deep acne cyst is considered practice of medicine. When a non-physician, such as an aesthetician or nurse is in effect practicing medicine by performing the initial diagnosis, treatment planning and administers prescriptive medical treatment without a physician on site or specific physician order or prescription, it is considered practice of medicine without a license which is being aided and abetted by the physician paid to serve as the (no show) Medical Director. The term “Medical Director“, is in fact not a clinical term but an administrative term. In the overwhelming number of “medspas” in Massachusetts which are owned by non-physicans some with questionable backgrounds, there is never a physician to see the patients at the time of diagnosis, treatment planning or complications. The term “Med” in medspa has been made obsolete by this reckless, greed-driven and dangerous practice. This type of practice is illegal in many states and should be illegal in all states to protect the unsuspecting public from risk to their health. The practice of medicine applies to any action that injures or modifies living tissue, this includes Acne which is a skin disease which may be misdiagnosed and mistreated by estheticians operating in Medspas without a doctor on site. Not all Acne is Acne Vulgaris (common). There are many dangerous imposters and many different causes or types which must have accurate diagnosis and careful treatment. Selling procedure packages and expensive products and peels is not the proper treatment and may be dangerous. Ultimately, who is responsible if there is a complication or delay in diagnosis of a serious condition resulting from services provided at a “medspa”? Who do you complain to if there is a problem? What is the medical director’s board-certification? How far away is the doctor’s own practice in case there is an emergency? How many other locations, especially independent “medspa”s does the doctor contract with? Never get LASER or injectable treatments in Salons or Studios by traveling nurses and rented equipment. What equipment is being used and who is responsible for maintaining it? Who is the provider of the malpractice liability insurance for the location and the doctor that is taking responsibility for your treatment? These are just some questions to ask BEFORE you get treatment. Its better to do your due diligence before consenting to a medical grade treatment with potentially serious complications than feign outrage after there is a complication. Each person is responsible for their own health when requesting elective procedures. Don’t let the medspa owner or medical directors’ greed get in the way of that.
