Deep acne scars such as ice pick and deep boxcar scars can be treated with the punch excision method. Using local anesthesia, the scars may be cut out with a scalpel and stitched together. The size of the tool depend from the size of the scar.The result is the formation of a linear scar, which in some cases may look better than the depressed scar. However, if the scar is still noticeable then it can be corrected using resurfacing methods such as Chemical peels, Dermabrasion or Laser resurfacing.

Excision is usually performed along with subcision. A benefit of this procedure is the absence of any epidermal injury, except for minute needle insertion points.

There are three options for acne scar excision:

1)Excision and direct skin closure: the scar is cut out and the surrounding skin is drawn together and stitched to form a new, neater scar line.

2)Excision and re-positioning: the scar is cut out and the direction of the scar is changed to run along a natural relaxed skin-tension line.

3)Excision and skin grafting: the scar is cut out and replaced with a skin graft or a flap from a healthy part of the body. Multiple lesions can be removed per session. Repeated sessions maybe required.
